Mrs. Cathy Hardekopf
Hello! My name is Cathy Hardekopf and I am the school nurse. I am excited to begin my 6th year working at St. Luke School. I grew up in a large family in west central Iowa. After high school I graduated with my associate degree in nursing from College of St. Mary and my bachelor’s degree in nursing from Creighton University both in Omaha, NE. Most of my career was spent in the hospital setting for 18 years before becoming a stay-at-home mom for 11 years. I was thrilled to be asked to take on the challenge of school nurse in 2018 at my home parish! Being able to do what I love and share my faith has been a blessing to me.
I am married to Charlie, and we have 3 children. In my spare time I love reading, baking, camping and gardening. I am excited to start another great year!

Kindergarten/ New Students 1st - 8th grade
Vision Screen form or Green Vision Card

Updated Immunization Record (with Tdap and menigococcal vaccines)
Other Forms:
Current Athletic Physical - For students grades 5-8 participating in school sponsored sports
Immunization Exemption Form (religious)
Immunization Exemption Form (medical)
Medication to be Given at School:
Please note -
The form above is required for medication to be given at school.
Over the counter medication must be in its original container and labeled with students name.
Prescription medication must be in a prescription bottle with a pharmacy label.

Keeping Our School Healthy:
When to keep your child home:
Fever of 100.0 F or higher
Vomiting
Diarrhea
Your child must be free of fever, vomiting and diarrhea for 24 hours without the use of Tylenol or Motrin before returning to school.
When a doctor’s note is needed:
If your child has been ill for 3 consecutive days.
If your child needs to be excluded from PE or recess for more than one day.
